~ All clients must fill out a “Client Intake” form to the best of your ability. This form will allow me to understand the current state or your hair and tailor your service to you; you’ll only need to fill this out once. ~All clients must also complete a series of three waiver forms to complete booking; details are listed in each form. You will receive a confirmation message once done. This must be done prior to appointment.

*Required to book extensions. This will give us a chance to meet and chat about your hair goals, and gives me the opportunity to see your hair in person to help decide what’s best for you.
